Required Prefiling Document List

Please use this list as a helpful guide to provide the required documents needed to get your case filed.

PROOF OF ID & SOCIAL SECURITY NUMBER

• Color copy of your DL or State Issued ID Card
• SS Card or a W-2 that lists your full SSN

CREDIT COUNSELING COURSE

• We recommend using Money Sharp because it’s $12 per household via this site:
  • https://www.moneysharp.org/bankruptcy-courses/credit-counseling-course/
  • Please take the FIRST Course called, “Credit Counseling”
• Please use Attorney Code: MRGFRESH

PROOF OF INCOME

• Bankruptcy law requires that you provide proof of your average monthly gross income from all sources for the last 6 months beginning with the first month before you file your case. Provide any of the following that apply to your situation:
  • Employee: 6 months most recent pay stubs
  • Self-Employed: Profit and Loss Statement
  • Other Fixed Income:
    • Retirement Statement
    • Pension Benefit Letter
    • Social Security Award/Benefit Letters
    • Disability
    • Unemployment, etc.

BANKING RECORDS

• For ANY account you are listed as owner or your money is deposited into. Please provide the 6 most recent monthly statements from:
  • Banks
  • Credit Unions
  • Other financial institutions:
    • Cash App
    • Venmo
    • PayPal
    • Apple Cash
• Please include ALL pages, including intentionally blank pages. 
• Transaction histories and screenshots of transactions are NOT accepted by the Trustees.

TAX RETURNS / TRANSCRIPTS

• Please provide a copy of the past 3 years tax returns or tax return transcripts (if you don’t have the returns themselves).
• You may download Tax Return Transcripts here: https://www.irs.gov/individuals/get-transcript
  • **Please note** Tax Account Transcripts will not work.
  • We must have Tax Return Transcripts please.

PROPERTY DOCUMENTS

• Vehicle Information:
  • Copy of ALL vehicle registrations (car, boat, trailer, RV, etc.)
  • Payoff quote for any vehicle liens
• Recent Statements for:
  • Stocks
  • Mutual Funds
  • IRAs
  • 401ks
  • Other Retirement accounts

LAWSUITS

• If you are being sued or have been sued, submit the first page of any lawsuits.
